1. Is your question about medical, life, dental, vision, prescription or pension?
   
   
2. What is the member’s social security number?
   
   
3. What company does/did the member work for?
   
   
4. Do you know the member’s group number?
   
   
5. Who is the patient and what is their relationship to the member?
   
   
6. What type of doctor or service are you asking about? Be specific! For instance, if  you ask what your Plan pays for a
   doctor’s office visit when you’re going to the Chiropractor, you may not find out that your Plan has a list of
   Chiropractors that you must use in order to have coverage.

Our Benefit Information Center in Alhambra and our offices in San Diego and Las Vegas now accept appointments. The Benefit Adviser will be more than happy to assist you with your Pension and Health & Welfare inquiries at your appointed time.

Appointments are scheduled between 8:30 a.m. and 3:00 p.m. (between 8:00 a.m and 4:30 p.m. for the Las Vegas office) Monday through Friday. Walk-ins are still welcome, but you can avoid the wait by making an appointment ahead of your visit. We can have your file information ready and waiting for you so that we can serve you more efficiently.
